Use this form if you are renting equipment (manikins, videos, etc) or renting and purchasing supplies for your class. While the Chapter strives to accomodate all of our instructors, 10 days notice for reserving equipment is appreciated.

NOTE: This form is to be used only by instructors currently authorized to teach the following programs:

  • Lay Responder First Aid/CPR/AED (2006 revision)
  • CPR/AED for the Professional Rescuer (2006 revision)
  • Bloodborne Pathogens
  • Water Safety instructors
  • Babysitter’s Training
  • Lifeguarding instructors
